Submit Student Immunization Form

Pursuant to Commonwealth of Virginia Law (Code of Virginia, Section 23.1-800), all incoming students are required to complete and submit a Student Immunization Form [PDF] prior to the beginning of classes. Failure to complete the Student Immunization Form will result in a registration hold and affect your ability to register for future classes. 

• To submit online: Upload to the secure Medicat website at https://Radford.medicatconnect.com. Choose Radford University as your college and log-in using your Radford University credentials. (An instructional video can be found at the bottom of this webpage.)

Email healthrecords@radford.edu with questions about the Student Immunization Form. Please make sure you include your full name, date of birth and student ID.

Deadline

Submit your Student Immunization Form prior to the start of classes. 

How to Upload Your Student Immunization

 

Required Immunizations

As outlined on the Student Immunization Form [PDF], incoming students are required to provide verification of the following immunizations, in addition to a Tuberculosis Self-Questionnaire*:

• Hepatitis B
• Meningococcal (ACYW-135)
• Measles, Mumps, Rubella (MMR)
• Tetanus Diphtheria (Td)(can be included with the Tetanus Diptheria Acellular Pertussis [Tdap])
• Poliomyelitis (OPV or IPV)
• *If you answer "Yes" to any questions on the Tuberculosis Self-Questionnaire, then you are required to complete and have a physician sign the Tuberculosis Screening, either on the Student Immunization Form [PDF] or using the Tuberculosis Screening Addendum [PDF].

Recommended Immunizations

While not required, the university recommends the following immunizations:

• COVID-19
• Serogroup B Meningococcal
• HPV, Quadrivalent or Bivalent (Age 26 or Under)
• Tetanus Diptheria Acellular Pertussis (Tdap)
• Hepatitis A
• Combined Hepatitis A + B Vaccine (note: Hepatitis B is a required vaccine)
• Pneumococcal
• Varicella
• Influenza (Flu)

Virginia Immunization Information System (VIIS) and Medicat

Medicat is connected to the Virginia Immunization Information System (VIIS).  Any immunization records in VIIS will post to Medicat.

Please note that this can result in your Highlander Portal enrollment checklist showing a Reviewed Non-Compliant status for the Student Immunization Form.

We strongly encourage you to log in to Medicat to see what immunizations are missing. Submit missing immunizations and/or the TB screening form directly to Medicat. 

Student Health Services

Student Health Services offers all required vaccines and TB testing. Student Health Services is located on the first floor of Moffett Hall on main campus. Learn how to schedule an appointment.

Waivers

Students with a medically necessitated waiver for any required immunizations should complete and submit Part III of the Student Immunization Form with a Healthcare Provider signature.

Students may elect to self-exempt from the Hepatitis B Vaccine and/or the Meningococcal Vaccine by completing Part III of the Student Immunization Form.

Students seeking a waiver on religious grounds must complete, including notarization, and submit the Commonwealth of Virginia Certificate of Religious Exemption form [PDF].

All waiver forms should be submitted in Medicat.
